Dear Bloomington High School Parents,

Please ensure your student's health-related consent form is submitted to their advisory teacher during advisory class by the end of the day on September 2, 2025. This form is mandatory for the school nurse to provide any health-related care to your student. Without this form, the nurse will be unable to see or assist your student with any medical needs.

Forms can be dropped off directly to your student’s advisory teacher during class. If you need a copy of the form or have any questions, contact the nurse’s office at (361) 333-8011 or email nurse@bloomingtonhs.org.
